Oregon Patient Safety Commission

The Oregon Patient Safety Commission (OPSC) was created by the Oregon Legislature in July 2003 as a "semi-independent state agency." OPSC's mission is to improve patient safety by reducing the risk of serious adverse events occurring in Oregon's healthcare system and by encouraging a culture of patient safety. National Patient Safety Foundation

The National Patient Safety Foundation (NPSF) partners with patients and families, the health care community, and key stakeholders to advance patient safety and health care workforce safety and disseminate strategies to prevent harm. Washington Patient Safety Coalition

The Washington Patient Safety Coalition (WPSC) collaborates with patient safety leaders across the state and the nation, sharing the latest developments and best practices to improve patient safety and reduce medical errors. The WPSC's mission is to improve safety for patients receiving health care in Washington, in all care settings. California Patient Safety Action Coalition

The California Patient Safety Action Coalition (CAPSAC) is a group of more than 60 of the state's leading public and private health care stakeholders who are dedicated to continuously improving patient safety by fostering the sharing of information. CAPSAC wants to change the current culture of blame and punishment in medical error reporting to one of trust and accountability. Its purpose is three-fold:

  • Provide opportunities for health care organizations to share best practices and learn from each other in order to prevent errors from re-occurring.
  • Educate regulatory agencies, legislators, consumers, health care providers, purchasers of health care and the media about research-proven approaches to increasing patient safety.
  • Serve as a resource to the Department of Public Health, the Center for Healthcare Quality, patient safety institutions, and professional licensing boards.
