With coordinated care, you will pick a PCP 360. A PCP 360 delivers full-circle care, coordinating your care with other providers, as needed. They are high-quality primary care providers (PCPs) who will partner with you and be accountable for your health. To see if your provider is a PCP 360, use our Find Care tool and look for the 360 icon under the provider's phone number.
With our Synergy Network, you get high-quality care close to home to cover all of Oregon, Southern Washington, and Idaho. The network consists of OHSU Health, Legacy Health, Adventist Health, Tuality Health and many more hospitals and providers.
