With coordinated care, your primary care provider is responsible for making sure you get all of the care you need — not just the care they can provide. If you need to see a specialist or go to the hospital, they will coordinate that care for you and work together with your other providers.
With traditional care, you are responsible for making sure you get all of the care you need. This includes the preventive services that will help you stay healthy. Also, providers most often work separately, which means that each may not know about all of your health history.
Moda Health is the only health plan that provides access to this kind of high-quality care.
With some Moda Health plans, you will also choose a PCP 360. A PCP 360 is a primary care provider who is part of a facility that has been certified by the Oregon Patient-Centered Primary Care Program or other similar programs. This means that a PCP 360 has to meet certain quality standards.
Ultimately, your PCP 360 will be accountable for delivering high-quality care that is centered on you.
After you have selected a Moda Health plan, each of your covered family members can pick the same PCP 360, or a different one — it's up to each of you.
