Our medical plans include a provider network that connects you with care in your hometown and across the country. As an OEBB member, your health plan includes access to this provider network.
This is one of the largest PPO networks in the state of Oregon and stretches from Southern Washington, Idaho, and into Northern California. It includes thousands of primary care physicians and specialists working together with Moda Health to help keep you healthy.
It is important to remember you may pay more for services from out-of-network providers than from in-network providers. Out-of-network providers may also bill you for the difference between your maximum plan allowance and their billed charges. This is known as "balance billing." In-network providers do not do this. See our plan summaries or your Member Handbook to learn more about in-network and out-of-network benefits and costs.
When you hit the road, care is never far. Our travel network comes with each medical plan in Oregon. When traveling outside of the Connexus service area you have in-network coverage through Aetna PPO Network with the Aetna Signature Administrators. You will use the travel network when:
Do you have a dependent living outside of the Connexus service area? If so, make sure to receive in-network benefits by updating your address in the myOEBB system. If any of your dependents live outside of the coverage service area, it is important to know what network to use to receive the in-network benefit level.
Dependents who live outside of the Connexus service area can still participate in Coordinated Care by selecting a PCP360. When they are in the Connexus service area, they will utilize their PCP360. When they are away, they will use their network as indicated above.

To nominate your medical provider, please fill out our medical provider nomination request form. Once completed, return the form to your provider's office, and let them know you would like them to initiate a network participation request with Moda Health.
