How to pay your premium
There are several ways you can pay your monthly premium:
Electronic Funds Transfer (EFT) from your bank
Access Moda Health eBill by logging in to Member Dashboard
Mailing a check
Send a check, payable to Moda Health Plan, Inc. to:

Make sure to put the member ID number for the account(s) to which you want the payment applied on the check.

Submitting a medical claim
If you need to request reimbursement for medical services, you may submit proof of payment and the itemized bill with this form.
Filing a complaint with Medicare
We work to resolve any issues you may have. You can also file a complaint directly with the Centers for Medicare & Medicaid Services (CMS) by using their online complaint form
