Moda Health's Medical Homes use the Summit and Synergy networks to connect your primary care provider with the rest of your care team (other providers, specialists, etc.), resulting in better quality and more affordable care.

What is a Medical Home?

Your Medical Home is the place you go for all of your health care needs. Your primary care provider works with you and the rest of your care team (other providers, specialists, etc.) to provide the best treatments for you.

This team-based approach to healthcare offers:

  • Personalized care centered on you
  • Faster, easier ways to get care
  • Support in meeting your health goals
  • Lower out-of-pocket costs with your Medical Home

If you have a plan with the Synergy or Summit network, you need to pick your Medical Home before using your benefits. There are many Medical Homes to choose from throughout the network.

Medical Home tier levels

The Synergy and Summit networks offer access to many recognized Medical Homes. This means that the provider has been certified by the Oregon Patient-Centered Primary Care Program. Moda Health's provider directory includes tier levels one through five. The higher the tier, the better coordinated the care. Each of your covered family members can pick the same provider, or a different one — it's up to each of you.

Log in to Member Dashboard

Members should log in to Member Dashboard at modahealth.com and proceed to the Medical Home tab.

Search by facility or provider name. Or use a ZIP code to find one near you.

Select the Medical Home

Select the Medical Home for you and any dependents under age 18.
Any dependents age 18 and older will need to do so in their Member Dashboard account.

Answers about selecting your Medical Home and finding care.

Do I have to select a Medical Home?

Not all plans require you to select a Medical Home. Only our Synergy and Summit plans require you to select a Medical Home before going in for care. Your Medical Home coordinates your care, including referrals to specialists or hospitals, as needed.

If you receive care before selecting a Medical Home, your claim(s) may be paid at the out-of-network benefit level.

Can I go to a primary care provider who is not in my Medical Home?

If you or a dependent receives care from a provider outside of the selected Medical Home, the claim(s) may be paid at the out-of-network benefit level.

How can I see a list of available Medical Homes?

We recommend that you log into your Member Dashboard account and go to the Medical Home tab to search for Medical Homes. But if you are not a member or don't have access to Member Dashboard, you can follow these simple steps:

  1. Go to our online provider directory, Find Care
  2. Choose your network: Synergy (Western Oregon) or Summit (Eastern Oregon)
  3. In the search criteria for Provider Type, choose "Medical Home Clinics & Medical Groups" to search by facilities or "Medical Home Providers & Doctors" to search by provider names.
  4. Enter your location — you may need to adjust the Distance option to expand the search area
  5. You may create a PDF of the list with the "Download/print results" link under the map.
