At Moda Health, our Disease Management program gives you the tools and guidance to take charge of your health with support from our certified health coaches. Our coaches help you build lasting healthy habits and make changes that move you towards feeling your best.
Health coaching provides you with a dedicated partner who offers guidance and support every step of the way. Your health coach will work closely with you to develop a personalized plan tailored to your unique needs, helping you make positive changes and achieve lasting results.
Our Health Coaching program is available at no cost to eligible members.
Health coaching doesn’t replace your medical care. It works alongside your doctor’s visits by helping you to:

Confidentiality
We take privacy seriously. All interactions with your Health Coach are completely confidential.
Flexible scheduling
With coaching hours available from 7:30 a.m. to 6 p.m. Pacific time, we strive to find a time that works for you.

When you join the Disease Management program, your health coach will help you set realistic goals, build confidence in managing your health, create healthy routines, and make handling your condition easier. You’ll meet by phone or video—whichever you prefer—over several months to build lasting habits and work through any challenges.

Claire enjoys partnering with members to cultivate mindsets and habits that promote greater well-being, whatever that means for each person. With a background in teaching and personal training, she has found her passion in supporting others on their journey of learning, personal growth and wellness. When she’s not coaching, you may find Claire exploring trails of the Pacific Northwest with her partner and dog, immersed in a good book or sharing delicious meals with friends and family.
Leah has a major love of learning. She holds a dual degree in politics and economics and a master’s degree in health education. After graduating, she earned a certification in applied positive psychology, health coaching as well as life coaching. Ultimately, she loves helping people answer the question: what makes you come alive? She particularly enjoys helping people manage (or change their relationship with) stress. When she is not coaching, you might have a hard time catching her because she’s either on the run (she loves being in motion and she loves to travel) or she has locked her doors, and she’s bunkered in with a good book and a mug of tea.
Since graduating from ASU's College of Health Solutions with a Bachelor of Science in Exercise and Wellness in 2017, Alex has been dedicated to the health and wellness field. Among his favorite coaching experiences is working exclusively with cancer patients to implement lifestyle modifications aimed at alleviating symptoms and cancer-related markers.
Outside of work, Alex enjoys spending time with his dog, Cooper, working out, playing video games and connecting with loved ones, as well as sharing an ever-timely Seinfeld quote.
Stacy has a diverse background, with degrees in social work and psychology, coupled with experience in personal training and group fitness instruction. Her passion lies in positively impacting others and providing empathetic coaching support to individuals navigating lifestyle changes, helping them create sustainable healthy habits.
Outside of coaching, Stacy enjoys camping, skiing, traveling, theater and quality time with loved ones over good food and good conversation.
Delaney's dynamic career in integrative nutrition, health coaching and sustainability reflects her passion for caring for both people and the planet. After earning her B.S. in Kinesiology and M.S. in Nutrition, she became a Registered Dietitian. Delaney enjoys incorporating discussions on body image and the relationship to food into her coaching work.
Delaney is also an actor, writer and outdoor enthusiast. Catch her on stage, scribbling away in a coffee shop or hiking in the mountains.
Sue is passionate about demonstrating that health extends beyond fitness and nutrition. With expertise in holistic health—covering mind, body and spirit—she believes true wellness includes mental and emotional well-being, empowering individuals to thrive and live their best lives.
Sue currently lives in Alaska with her husband. In her free time, she enjoys traveling, spending time with her grandkids, hiking, cross-country skiing, kayaking, paddleboarding and so much more!
Winter holds a Bachelor of Exercise Sports Science, which has opened purposeful opportunities for her as an educator, coach, community leader, and mentor. She is dedicated to helping others improve and become the best versions of themselves. In her free time, she enjoys mentoring and training student athletes alongside her husband through an organization she founded in 2008, called the Jewel Athletic Scholastic Institute (JASI).
She loves traveling, whether it’s within the country or abroad, and cherishes the priceless moments spent with family, creating lasting memories.
